LVN - Hospice

Provides services in accordance with the policies/procedures of Hospice. Assists with the implementation and evaluation of the care of assigned patients. Provides patient care as directed by the attending physician and in collaboration with health team members. The LVN is directly responsible to the supervising RN. Will also be required to rotate being on call during week and weekends for the department
